logistics and statement on insurance

  • Payment accepted: Cash, Card, Check, HSA, and Venmo.

  • Insurance Reimbursement and Utilization:

    • I am an In-Network (INN) provider via membership with Alma and credentialed with Aetna, Cigna, and Optum plans. If you elect to utilize your insurance, we will check your eligibility and benefits via the Alma platform linked here. Please be aware that you may be invoiced for any copays, may need to meet your deductible, may be denied and are ultimately responsible for covering services.

    • As an Out-of-Network (OON) provider with most insurance plans, I am able to provide receipts for each session or a superbill with diagnostic codes that may be accepted by your insurance company for out-of-network reimbursement to you based on the terms of your plan.

  • Reduced Rates: I maintain a limited number of reduced rate spots each year (reassessed annually) for single parents, social workers in the field, students, and teachers determined by an income-based sliding scale. Please inquire as needed.

Cancellation Policy

Clients are free to cancel or change a scheduled appointment provided a 48-hour notice has been given.

Cancellations within the 48-hour window will be charged at the full rate of a standard session, with the exception of an emergency.

no surprise act notice

Under the No Surprises Act, health care providers need to give patients who do not have insurance or who are not using insurance an estimate of the bill for medical items or services.

As a client, you have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs to medical tests, prescription drugs, equipment, and hospital fees. My services do not provide or include medical tests, prescription drugs, equipment, or hospital fees.

Make sure your therapist gives you a Good Faith Estimate in writing at least one business day before your medical service or at the time of your appointment. You can also ask your therapist, and any other provider you choose for a Good Faith Estimate before you schedule an item or service.

If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Make sure to save a copy or picture of your Good Faith Estimate.

For questions or more information about your right to a Good Faith Estimate, visit www.cms.gov/nosurprises or call (323) 609-9887.
